Immunization Information for Parents
As we prepare for an exciting new school year, we want to remind all parents and guardians that, per Georgia state law (O.C.G.A. § 20-2-771), no child may be admitted to or attend school without a current Certificate of Immunization (Form 3231) on file — or an approved exemption.
What You’ll Need to Submit:
A valid GA Certificate of Immunization (Form 3231)
OR a medical exemption (noted on Form 3231)
OR a notarized religious objection affidavit
Take advantage of the summer months to get this important requirement completed. Documentation must be submitted for enrollment or attendance. If you have a regular medical provider, please call them to schedule an appointment or take advantage of the following immunization clinics being offered in our community:
Good Samaritan: Thursday, July 24th, from 9 AM to 7 PM. Call 706-253-4633 to schedule an appointment.
Health Department: Tuesday, July 29th, from 1 PM to 6 PM, offering immunizations and 3300 screenings. Call 706-253-2821 for more information.
You can submit your immunization documents at the Board of Education this summer, during your school's open house, or by emailing them to registration@pickenscountyschools.org.
