Immunization Information for Parents

As we prepare for an exciting new school year, we want to remind all parents and guardians that, per Georgia state law (O.C.G.A. § 20-2-771), no child may be admitted to or attend school without a current Certificate of Immunization (Form 3231) on file — or an approved exemption.

What You’ll Need to Submit:

  • A valid GA Certificate of Immunization (Form 3231)

  • OR a medical exemption (noted on Form 3231)

  • OR a notarized religious objection affidavit

Take advantage of the summer months to get this important requirement completed. Documentation must be submitted for enrollment or attendance. If you have a regular medical provider, please call them to schedule an appointment. 

You can submit your immunization documents at the Board of Education this summer, during your school's open house, or by emailing them to registration@pickenscountyschools.org.
